A quiet but significant shift is underway in public perception and policy regarding cannabis. As legalization expands, a narrative often emphasizing its relative safety compared to opioids has taken root.

While it is true, as epidemiologist Deborah Hasin of Columbia University notes, that “people don’t die from cannabis overdose,” this distinction should not be mistaken for a clean bill of health. A growing body of scientific evidence reveals a complex and sometimes troubling profile of risks associated with regular use, challenging the notion of cannabis as a benign substance and highlighting consequences for both the mind and body that demand a more nuanced public conversation.

The impact of cannabis on the brain, particularly with regular use, extends far beyond temporary euphoria or relaxation. One of the most severe and alarming risks is the induction of substance-abuse psychosis—a temporary state where individuals may experience delusions, paranoia, and auditory hallucinations, losing touch with reality. Disturbingly, research indicates the risk for this acute condition is higher with cannabis than with cocaine.

While typically resolving within days, these episodes can require hospitalization. More concerning is the long-term trajectory; up to one-third of individuals who experience this cannabis-induced psychosis may go on to develop schizophrenia, a permanent and debilitating mental health disorder. The risk is not confined to psychosis. Observational studies consistently link frequent cannabis use to increased rates of depression and heightened aggression, such as violence among dating couples.

For the developing adolescent brain, which is undergoing critical maturation and “pruning,” these effects are profoundly magnified. Teenagers who use cannabis regularly face a 37 percent increased likelihood of developing depression by young adulthood and demonstrate higher rates of suicide, underscoring a vulnerable population at significant risk.

The consequences of regular cannabis use are not merely psychological; they extend directly to physical health, with the cardiovascular system emerging as a primary area of concern. A major study published in the Journal of the American Heart Association found that regular users face a substantially higher risk for heart attack and stroke, with rates rising by 25 percent and 42 percent, respectively.

This cardiovascular strain is believed to occur because THC, cannabis’s primary psychoactive compound, affects blood flow and interacts with receptors embedded throughout the cardiovascular system. The method of consumption adds another layer of risk; smoking cannabis introduces particulate matter that further jeopardizes heart health.

Beyond the heart, regular use can lead to paradoxical physical syndromes. While cannabis is famed for curbing chemotherapy-induced nausea, it can also trigger a debilitating opposite effect known as cannabinoid hyperemesis syndrome, a condition marked by cyclic, severe vomiting that is difficult to control. For pregnant individuals, use is associated with higher risks of preterm birth and dangerously low birth weights, posing serious questions about prenatal exposure.

This evolving evidence paints a picture far more complicated than the simple binaries of “safe” or “dangerous.” It reveals a substance with a distinct risk profile, where the absence of lethal overdose does not equate to an absence of harm.

The data calls for a clear-eyed public health message that balances the documented medicinal benefits for specific conditions against the significant psychological and cardiovascular dangers of non-medical, regular use. This is especially critical for young people, whose developing brains are uniquely susceptible to long-term alteration.

As society navigates this new landscape, moving beyond the simplified comparisons to opioids is essential. The true task is to foster an understanding of cannabis on its own terms—acknowledging its potential for harm, protecting vulnerable populations, and ensuring that personal and policy decisions are informed by the full spectrum of scientific evidence, not just its most forgiving comparisons.
